To request a History Box, please complete the form below. Each school may reserve one box per session, processed on a first-come, first-served basis. A representative of the Canadian Museum of History will contact you with the status of your request.

To help us better respond to your request, please include your spring break dates in the comment section so we can work around them whenever possible.
